Job Summary
Home Care Direct Support Professionals provide day-to-day care and support to clients in their homes, including personal care, medical monitoring, transportation, meal preparation, and adherence to person-centered plans. They lead by example, supervise and train direct support staff, maintain meticulous documentation (HPC care notes, behavior logs, incident reports, medication records, mileage), and collaborate with supervisors to ensure quality and continuity of care. They help clients meet personal goals, ensure a safe environment, and may transport clients to activities or appointments. Roles require a high school diploma or equivalent, 21+ years of age, and strong communication; CPR/First Aid is preferred. Flexible scheduling including overnight shifts is common, and benefits such as 401(k) and PTO are offered to eligible teammates.
Required Qualifications
- High school diploma or equivalent
- At least 21 years of age
- Prior experience with seniors and/or adults with special needs preferred
- Valid Driver’s license and reliable transportation for driving duties
- Proof of vehicle registration and insurance for driving duties
- C.P.R. and First Aid certificate preferred
- Strong communication and interpersonal skills
- Ability to work a flexible schedule including overnight for 24/7 clients
- Ability to bend, pull, push, squat, turn, transfer, and lift 50 pounds
- Ability to stand for extended periods while providing physical assistance
