Job Summary
Case Manager provides community- and/or office-based case management to patients and families, assisting in planning, connecting, outreaching, advocating, coordinating, and monitoring needs; helps patients access health services, financial aid, employment, education, social services, and natural supports; develops and monitors individualized treatment plans and documents services; supports recovery and a safe environment for clients; supports a no-wrong-door approach across multiple behavioral health programs in Arizona; expressed emphasis on outreach, coordination, and access to services. Responsibilities include assessment of patient needs, service coordination, and documentation in alignment with recovery-oriented care and CBI policies; requires engagement with GMH/SU population and adherence to regulatory requirements.
Required Qualifications
- Highschool diploma or GED required
- Associates degree or higher in a field related to behavioral health preferred
- 1-3 years of experience working with GMH/SU population
- 1-3 years of experience in behavioral health position providing outreach and engagement activities
- 1-3 years of experience in a medical setting preferred
- Minimum of 6 months of recovery from substance use and/or mental health disorders preferred
- Arizona Fingerprint Clearance Card
- CBI policies and procedures compliance
- Valid Identification required
- Current AZ Driver’s License (valid and in good standing) preferred
- Clean 39-month driving record preferred
