Patient Transition Coordinator

Job Summary
Coordinate patient transitions to home health, hospice, home infusion, and palliative services; organize and verify complete medical records and start of care; collaborate with Care Transition Coordinators and intake teams; facilitate referrals and outside agency placement when needed; manage large volumes of information with attention to detail; deliver exceptional patient and family service; proficiently use MS Word, Excel, Access, and PowerPoint; ensure compliant documentation and communications; support enrollment and transition processes across post-acute care settings.
Required Qualifications
- Bachelor’s degree in a health-related field preferred
- An equivalent combination of education and experience will be considered
- Minimum of one (1) year of experience in a related health care setting preferred
- One (1) year experience in home health, hospice and/or palliative preferred
- Prior experience with electronic medical records preferred
