Onboard patients into the Type 2 Diabetes Reversal and Metabolic Health Programs, responding to inquiries to ensure access to appropriate services and resources. Provide dedicated support during onboarding to facilitate a seamless transition, while fostering relationships that establish LifestyleRx as the trusted provider for the patient's reversal journey. Assist practitioners with appointment cancellations and rescheduling, and moderate group medical visits by monitoring chats, presenting weekly video content, and supporting connectivity issues. Follow up with clinics and labs to resolve communication problems, missing results, or referrals. This role supports the team's team-based approach to precision lifestyle medicine, combining medical expertise with nutritional coaching and digital tracking.
