Job Summary
Manage patient schedules, edit for agent availability, and update the worker console while initiating infection control forms and maintaining the client hospitalization log. Process discharge visits, OASIS collections, and rescheduled appointments, then prepare weekly agent schedules for director approval. Verify visit notes, run scheduling reports, and assist with internal patient transfers. If clinical, review lab reports, fax to physicians, and route to medical records; perform patient visits or participate in on-call rotation as needed. Requires LPN/LVN licensure, one year of home health experience, and a valid driver's license.
Required Qualifications
- Licensed Professional Nurse or Licensed Vocational Nurse licensed in the state in which he / she practices
- At least 1 year of home health experience
- Valid state driver's license
- Automobile liability insurance
- Currently licensed in the State of employment if applicable
- Excellent communication skills
- Ability to interact well with a diverse group of individuals
- Strong organizational skills
- Ability to manage and prioritize multiple assignments
Desired Qualifications
- Prior packet review / QI experience
- Coding certification
