Patient Services Coordinator LPN

Manage patient schedules, including supervisory visits, discharge visits, OASIS collections, and transfers to inpatient facilities. Reschedule declined or missed appointments, prepare weekly agent schedules, and run scheduling reports for missed visits. Assist with weekly case conferences, receive clinical questions from field staff, and refer issues to the Branch Director as needed. Perform patient visits or participate in on-call rotation while maintaining hospitalization logs and distributing lab reports. Verify visit paper notes in the scheduling console and assist with internal patient transfers between branch offices.
