Patient Services Coordinator Home Health - Full-time

Job Summary
Schedule patients to branch field clinicians and communicate with field staff, patients, physicians, referral sources, caregivers, and service providers to maintain care coordination and continuity. Manage the on-call notebook and hospitalization logs to enhance communication among stakeholders. Maintain a valid state driver's license, automobile liability insurance, and dependable transportation in good working condition. Must possess basic technology skills including mobile device operation. Requires a high school diploma, plus either an active clinical license with one year of healthcare experience or one year of home health, hospice, or pediatric experience. Required Qualifications
- high school diploma or equivalent
- licensed practical or vocational nurse in the state in which they currently practice, with at least one year of clinical experience in a healthcare setting
- at least one year of home health, hospice, or pediatric experience within the last 24 months, and have a demonstrated understanding of staffing and scheduling requirements related to home care services
- basic demonstrated technology skills, including operation of a mobile device
- valid state driver license
- automobile liability insurance as required by law
- dependable transportation in good working condition
- be able to safely drive an automobile in all types of weather conditions
