Job Summary
Schedule patients to branch field clinicians and communicate with staff, physicians, and caregivers to maintain care coordination and continuity. Manage the on-call notebook and hospitalization logs to enhance communication among stakeholders. Maintain a valid state driver's license, automobile liability insurance, and dependable transportation in good working condition to safely drive in all weather types. Requires a high school diploma, an LPN/LVN license with clinical experience, or one year of home health/hospice/pediatric experience with demonstrated staffing knowledge. Basic mobile device technology skills are required.
Required Qualifications
- high school diploma or equivalent
- licensed practical or vocational nurse in the state in which they currently practice
- at least one year of clinical experience in a healthcare setting
- at least one year of home health, hospice, or pediatric experience within the last 24 months
- demonstrated understanding of staffing and scheduling requirements related to home care services
- basic demonstrated technology skills, including operation of a mobile device
- valid state driver license
- automobile liability insurance as required by law
- dependable transportation in good working condition
- ability to safely drive an automobile in all types of weather conditions
