Job Summary
Scheduling visits and coordinating care for home health patients; communicates with field staff, patients, physicians; manages staff schedules; handles infection-control forms; provides back-up during lunch hours; assists with weekly case conferences; refers clinical questions to Branch Director; maintains hospitalization and coordination logs; ensures visits and add-ons are scheduled per case conference outcomes; generates scheduling reports and distributes weekly agent schedules; reviews weekly productivity and geographic issues for approval by Branch Director; may perform patient visits if clinical; supports internal transfer of patients between branch offices; processes lab reports and routes to Medical Records when needed.
Required Qualifications
- Must possess excellent communication skills
- Must have at least 1 year of home health experience
