Job Summary
Patient Services Coordinator in Home Health responsible for scheduling visits, coordinating care between patients, field staff, physicians, and office teams; manages staff schedules, reassigns visits when needed, handles on-call support during busy periods, updates availability in the scheduling console, and supports case conferences. Roles include processing infection-control forms, generating and routing Infection Reports, coordinating add-ons and discharges (including OASIS collection and recert visits), ensuring supervisory and nurse/injection visits are scheduled, generating weekly agent schedules, and performing initial productivity reviews for geographic issues. Requires strong communication, organizational skills, and at least 1 year of home health experience; coding certification preferred. Pay range is $40,000 - $52,300 per year for full-time employment; benefits include medical, dental, vision, 401(k), paid time off, disability and life insurance, and other programs.
