Job Summary
Join The Care Team as a Patient Services Coordinator in Decatur, IL, supporting hospice patient care through scheduling and coordination of evaluations, admissions, routine visits, visit reports, and documentation in the EMR HCHB, with a Monday–Friday 8:00 AM–4:30 PM schedule and in-office work. The role emphasizes coordinating schedules, ensuring timely visits, maintaining accurate clinical notes, and communicating with patients, families, and care teams. Requires healthcare scheduling/coordination experience, strong organizational and multitasking skills, experience with HCHB/Homecare Homebase preferred, and a current driver’s license plus ability to pass background, MVR, TB testing, and CPR certification as required.
Required Qualifications
- Active driver’s license
- Ability to pass a criminal background check
- Ability to pass a Motor Vehicle Record (MVR) check
- Current TB testing in accordance with agency policy
- Current CPR certification as required for the role
