Job Summary
Independent contractor Patient Service Representative for ZOLL LifeVest program. Train patients and caregivers on LifeVest use, manage equipment inventory, schedule fittings, and conduct follow-up visits. Travel to patients' homes for fittings or follow-up, train patients and caregivers, obtain Patient Agreement and WEAR Checklist, and fax documents to ZOLL within 24 hours. Requires 1 year of professional patient care experience, professional patient care resume, valid driver's license and car insurance, and willingness to incur training/credentials. Flexible evening and weekend availability; assignments can be accepted or declined based on schedule.
Required Qualifications
- Have 1 year patient care experience
- Patient experience must be professional (not family caregiver)
- Patient experience must be documented on resume
- Disclose personal NPI number (if applicable)
- Have a valid driver's license and car insurance
- Willing to pay for additional training and vendor credentials (i.e. DME/RepTrax), fees not paid by ZOLL
