Job Summary
Independent contractor Patient Service Representative trains patients and caregivers on LifeVest device, sets up the equipment, and trains patients on use and care. Responsibilities include contacting caregivers/family to schedule services, traveling to patients' homes and health care facilities, training patients and caregivers, programming LifeVest per physician orders, measuring garment size, obtaining signatures for paperwork, transmitting signed agreements and wear checklists to ZOLL within 24 hours, managing device and garment inventory, and disclosing any potential referral sources. Requires 1 year of paid patient care experience, ability to schedule flexible assignments (daytime/evenings/weekends), background check, valid ID/driver’s license, and willingness to incur occasional credentialing and DME fees. Florida/Ohio candidates require level 2 screening.
Required Qualifications
- Have 1 year patient care experience
- Patient experience must be in a paid professional environment (not family caregiver)
- Patient experience must be documented on resume
- Completion of background check
- Florida and Ohio candidates must complete a level 2 screening
- Disclosure of personal NPI number (if applicable)
- Valid driver’s license and car insurance and/or valid state ID
- Willingness to pay a $30 annual DME fee which is deducted from a completed Work Order
- Willingness to pay for additional vendor credentialing (i.e. RepTrax) if needed geographically
