Job Summary
LifeVest Patient Service Representative: train patients and caregivers on LifeVest wearables as an independent contractor. Responsibilities include managing equipment and garment inventory, scheduling fittings with caregivers/family, traveling to patients' homes for fittings or follow-ups (evenings/weekends), obtaining/signing of the Patient Agreement and WEAR Checklist, and faxing documentation to ZOLL within 24 hours of assignment. Qualifications emphasize patient care experience (1 year), professional presentation, documented experience on resume, and a valid driver's license with car insurance. The role involves program equipment per physician orders, measuring patient garment size, training patients and caregivers, and coordinating follow-ups.
Required Qualifications
- Have 1 year patient care experience
- Patient experience must be professional (not family caregiver)
- Patient experience must be documented on resume
- Disclose personal NPI number (if applicable)
- Have a valid driver's license and car insurance
- Willing to pay for additional training and vendor credentials (i.e. DME/RepTrax), fees not paid by ZOLL
