Job Summary

Schedule clinical visits for a multi-disciplinary team of RNs, MSWs, LPNs, and Aides using a computer-based scheduling program. Coordinate schedules considering caseload capacity, continuity of care, staffing levels, and geographic areas to maximize efficiency and safety. Make adjustments to patient visits when staff call in sick, are on vacation, or assigned low census. Review daily patient visit records, edit and move visits based on clinical abilities and location, and complete accurate data entry into the EMR. Communicate scheduling needs and coverage changes to managers and clinical staff, and answer incoming telephone calls to appropriately address caller needs. Participate in quality improvement activities, staff meetings, and in-service training sessions, and maintain confidentiality of all patient and ministry information.

Required Qualifications

  • Fluency in English
  • Strong verbal and written communication skills
  • Ability to read, analyze, and interpret business periodicals, professional journals, technical procedures, and governmental regulations
  • Ability to write reports, business correspondence, and procedure manuals
  • Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties
  • Proficiency in Microsoft Office (Word, Excel, Outlook)
  • Ability to type proficiently and accurately enter data into databases
  • Ability to use standard office equipment including computers, telephones, copiers, and fax machines
  • Strong organizational skills
  • Detail-oriented with ability to meet deadlines
  • Critical thinking, problem-solving, and troubleshooting capabilities
  • Ability to prioritize multiple tasks, adapt to changing conditions, and meet productivity and quality standards
  • Demonstrated knowledge of general office practices and phone etiquette
  • Teamwork and interpersonal skills, including the ability to maintain positive, collaborative relationships
  • Ability to follow appropriate communication channels and practice effective teamwork
  • Commitment to integrated healthcare at home philosophy
  • Knowledge of scheduling systems
  • Flexibility to adapt to changing clinical schedules and patient needs
  • Regular standing, walking, and manual dexterity
  • Ability to lift and move objects up to 50 pounds
  • Visual acuity including close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus
  • Routine use of standard medical equipment and tools associated with clinical care

Desired Qualifications

  • 1 year Previous experience in scheduling or in clerical/data entry position using spreadsheets and databases
  • Experience in health care, hospice or home health industry
  • Familiarity with spreadsheets, word processing, and healthcare databases
