Patient Safety Consultant

Patient Safety Consultant responsible for leading and supporting patient safety investigations, applying safety science methods (FMEA, RCAs, SWARM, PDSA), and driving proactive risk-reduction across a pediatric care organization. You will evaluate safety reports, conduct interviews and medical record reviews with psychological safety, perform gap assessments, design system-level actions, and partner with clinical stakeholders to implement changes. The role emphasizes data-driven analysis, proactive risk assessment, and fostering a just, restorative culture while delivering education to clinicians and leaders on high reliability principles, resilience engineering, effective communication, and sustaining situational awareness. Collaboration across clinical, operational, and leadership teams to reduce preventable harm and support quality and safety initiatives is core, with responsibilities including data analysis, reporting, and participation in safety committees.
