Job Summary
Patient Relations and Retention Liaison supports the Senior/Medicare population and AltaMed Plus product by conducting telephonic targeted outbound engagement, welcoming patients at clinic for initial appointment, coordinating introductions with the clinical team, guiding patients through opt-in options for AltaMed Plus, and providing ongoing service and care coordination insights. Responsible for follow-up with identified patients/prospects, monitoring patient experience, and collaborating with clinics and AM+ teams to maintain a cadence of touchpoints throughout the program lifecycle. Requires knowledge of health plans/benefits, ability to discuss AltaMed services, and proficient use of CRM and EPIC; bilingual English/Spanish is required; role combines time in clinic and corporate office with emphasis on patient engagement, advocacy, problem resolution, and service recovery.
Required Qualifications
- High school diploma required
- AA/BA in healthcare (or related field) is a plus
- Minimum of 3 years working in a health care environment
- Knowledge of plan benefits for Medicare and/or senior market
- 3 years experience in health care coordination services, patient experience, and retention efforts with ethnically diverse populations
- Bilingual English/Spanish proficiency
- Experience with CRM and EPIC is a plus
