Job Summary
Observe assigned patients and maintain visual contact, quickly responding to care needs while ensuring safety and comfort. Perform basic clinical services including assisting with activities of daily living, vital signs, blood sugar checks, and toileting. Provide 1:1 care for patients at risk of suicide, adhering to strict precautions, inventory checks, and diversional techniques. Document observations in the EMR every 15 minutes for suicide cases and hourly for safety, reporting status changes to licensed staff. Transport non-monitored patients and supplies within the facility while maintaining confidentiality.
Required Qualifications
- High school diploma
- Must be able to maintain visual contact with assigned patient
- Must be able to quickly respond to patient care needs
- Must be able to provide care for patients at risk of suicide
- Must be able to follow suicide precautions protocol
- Must be able to remain within close proximity of the patient at all times
- Must be able to utilize Desescalation techniques
- Must be able to provide diversional activities
- Must be able to provide basic nursing care
- Must be able to assist assigned patients with activities of daily living
- Must be able to document in the EMR appropriately
- Must be able to perform Body Mechanics
- Must be able to assist with Bathing
- Must be able to assist with Feeding
- Must be able to perform Aspiration Precautions
- Must be able to track Inputs and Outputs
- Must be able to assist with Toileting
- Must be able to perform Peri-Care
- Must be able to perform Oral Care
- Must be able to assist with Ambulation
- Must be able to perform Bed-Making
- Must be able to assist with Range of Motion
- Must be able to manage Restraints
- Must be able to use PPE
- Must be able to take Vital Signs
- Must be able to perform Blood Sugar Checks
- Must be able to recognize pulseless patients

- Must be able to document patients' activity every 15 minutes for suicide patients
- Must be able to document patients' activity every 1 hour for safety patients
- Must be able to provide appropriate documentation on patient record
- Must be able to assist with patient quality outcomes
- Must be able to demonstrate effective communication skills
- Must be able to appropriately report patient observations and changes in patient status to licensed staff
- Must be able to maintain confidentiality of patient information
- Must be able to report any potentially dangerous behavior or concerning conversations to primary RN
- Must be able to transport non-monitored patients and supplies
- Must be able to transport specimens
- Must be able to transport equipment within facility
- Must be able to review medical record for orders
- Must be able to check if appropriate suicide precaution orders are in
- Must be able to assist with customer service
