Job Summary
Support patient navigation efforts by providing telephonic and in-person support to patients facing barriers to accessing health care, including home visits and outreach in diverse settings. Serve as a liaison between individuals, communities, and health services to facilitate care access, improve cultural responsiveness, and address social determinants of health. Work under the Women's Clinic Administrator to connect clients with Medicare, Medicaid, and special needs resources while utilizing electronic medical records. Obtain Community Health Worker Certification within the first month of employment. Fluent in English and Spanish is preferred.
Required Qualifications
- Experience working with Medicare, Medicaid or Special Needs populations
- Ability to connect with people and understand the challenges they face
- Ability to use a range of outreach methods to engage individuals and groups in divers settings
- Well connected to the community and resources within the community they will serve
- Effective written and verbal communication skills demonstrating respect and cultural awareness during interactions with patients
- Ability to travel throughout the assigned region and comfort with conduction home visits
- High school degree or equivalent
- Be able to obtain Community Health Worker Certification within first month of employment, if not currently certified as a Community Health Worker
- 1+ years of experience in a physician's office or clinical setting
- Proficient computer skills to work efficiently with electronic medical records
