Job Summary

Conduct direct, field-based services to enrolled consumers with HIV/AIDS, including visiting between clinic visits to reinforce treatment plans, provide appointment reminders, and deliver individual health education. Accompany consumers to primary care, specialty care, and social service appointments while providing Directly Observed Therapy (DOT) services to monitor medication intake and compliance. Support implementation of comprehensive Service Plans by assisting with securing housing, medical care, nutrition, and transportation, as well as handling relevant applications and paperwork. Record all service provision daily in AWARDS Case Management Software and collect Primary Care Status Measures data. Participate in daily planning meetings, case conferences, and quality assurance activities.

Required Qualifications

  • Associate's Degree
  • Effective written and oral communication skills

Desired Qualifications

  • Experience working with people living with AIDS
  • Experience working with homeless people
  • Experience working with substance users
  • Bi-lingual Spanish
  • Experience working within an outreach model
  • Experience working within a casemanagement model
  • Basic computer skills (e.g. MS Office suite and Windows)
