Job Summary

Act as a patient advocate by coordinating appointments with healthcare professionals and assisting patients in understanding their diagnosis and treatment options. Assess the physical, functional, social, mental, environmental, and financial needs of clients to plan, implement, and evaluate services for cancer patients. Provide emotional support and counseling while navigating the healthcare system, including insurance issues and necessary paperwork. Identify and develop new community resources, monitor service delivery, and prepare clear reports. Maintain accurate patient records ensuring confidentiality. Act as the formal and informal support system for patients and families.

Required Qualifications

3+ years working in a social services environment
Must be able to manage and coordinate client needs
