Job Summary
Bridge the gap between clinician and patient by providing individual support and assistance in navigating the health care system to ensure continuity of care. Call new patients within 24 hours of their visit to check in regarding follow-up appointments and needs, while working with the RN Navigator disease team to manage scheduling, pre-cert processes, follow-up calls, and record retrieval. Participate in daily multi-disciplinary rounding on the inpatient oncology unit to identify coordination needs and assist patients in transitioning from inpatient to outpatient areas without service interruption. Track all patient contacts, follow-ups, barriers to care, and outcomes, and connect patients to additional services such as social work and financial counseling. Ensure the delivery of patient care through the coordination of customer services and interdepartmental activities to support quality, cost-effective outcomes for the oncology population.
Required Qualifications
- Experience working in a healthcare office setting
- Experience working in community outreach and education
- Knowledge of health care
- Knowledge of inpatient settings
- Knowledge of outpatient settings
- Knowledge of Oncology specific patient population
