Job Summary
Connect patients to relevant community resources while collaborating with family members, providers, and staff to enhance health, decrease readmissions, and increase satisfaction. Partner with the VP of Physician Services under direct supervision to manage relationships with clinic, hospital, and specialty resources. Utilize strong verbal communication and active listening to support patients and caregivers effectively. Requires three years of customer service experience, high school diploma or GED, and ability to multi-task and prioritize. Evening part-time schedule at OakBend Medical Center.
Required Qualifications
- High School Graduate or GED Equivalent
- 3 years previous customer service experience
- Previous experience in a customer support role
- Strong phone and verbal communication skills
- Active listening

- Ability to multi-task, priorities, and manage time effectively
- Ability to establish and maintain strong working relationships with staff and management
