Job Summary
Conduct patient needs assessments, coordinate care transitions, and facilitate referrals to medical, behavioral health, and community support programs. Perform weekly check-ins, appointment reminders, and transportation assistance while documenting navigation activities in the EHR within 72 hours of encounters. Connect patients to housing, food, and resource services, ensuring 90% of referrals are successfully linked within 14 days. Collaborate with healthcare providers, pharmacies, and social agencies to maintain coordinated care and support 85% patient satisfaction rates. Work with the Clinical Case Manager and Integrated Behavioral Health Consultant to address social determinants of health and close clinical measure gaps.
Required Qualifications
- High school diploma or equivalent
- Associate's degree, bachelor's degree or higher
- Experience navigating and working in a clinical setting
- Willingness to work in a team environment
- Willingness to work in community setting at outreach events
- Valid and unrestricted Arizona driver's license (within 30 days of hire if applicant is from out-of-state) with no DUI/DWI or reckless driving convictions in the last five years
- No more than two at-fault accidents in the last three years
- Ability to maintain a valid Arizona driver's license during employment
- Ability to be insurable under NATIVE HEALTH's liability auto policy
- Pass a criminal background check and obtain a Class I Fingerprint Clearance Card within the initial ninety (90) calendar days of employment
- Ability to retain a valid Class I Fingerprint Clearance Card while employed with NATIVE HEALTH
- Pass substance abuse testing upon employment
- Ability to submit to random testing during employment
- Provide an updated immunization (IZ) record at the time of employment
- Current (within 12 months) Tuberculosis (TB) skin report upon employment
- Ability to provide an annual update to the TB skin report
- Obtain Basic Life Support (BLS) certification within (90) calendar days of employment
- Ability to maintain BLS certification during employment
Desired Qualifications
- Preferred certified phlebotomist or willing to become certified
- Bilingual Spanish speaker
- Knowledge of the cultural and social characteristics of the Native American and ethnic minority populations in Maricopa County
- Maintain knowledge of community resources and programs along with eligibility criteria
- Knowledge of all Native Health Programs and services
- A willingness to go above and beyond to meet patient needs
- Knowledge of the economic and health constructs for Native Americans in Maricopa County
- Culturally responsive to all backgrounds
- Must possess excellent communication skills
- Ability to manage multiple tasks and cases at one time
- Be detailed oriented professionally
- Be an effective leader who can work well with other health care personnel
- Interact in a culturally responsive way with patients and their families
- Ability to effectively function as a team member as part of Native Health
- Work collaboratively with healthcare providers, pharmacies, insurance representatives, and social service agencies to ensure coordinated care
