Patient Navigator Manager

Supervises the Consumer Navigation process to ensure proper protocols are followed, emphasizes continuous improvement and teamwork, and provides ongoing feedback with focused action steps for areas of improvement. Completes review, data submission, and reporting activities timely based on consumer referral data and creates action plans to improve outcomes. Rounds on staff to understand service perception and provides feedback to enhance patient navigation processes. Addresses service issues and collaborates with departments for timely resolution. Drives customer service initiatives by enhancing care navigation processes. Works with clinical leaders hospital-wide and collaborates on physician onboarding and operational processes. Participates in hospital committees and integrates hospital protocols into daily activities. Ensures compliance with state, federal, and other regulatory requirements. Develops and maintains policies, procedures, and standard work for oversight. Demonstrates strong organization, analytical, and problem-solving skills, with excellent communication abilities; proficient in Microsoft Office (Outlook, Word, Excel); able to lead cross-functional teams and implement best-practice processes for consumer navigation. Education: Bachelor's preferred; Experience: 2+ healthcare-related (preferred), 5+ customer service (required); Certifications: Six Sigma certifications preferred.
