Job Summary

Guide community members through diagnostic evaluations and maintain communication between patients, families, clinicians, and the healthcare system. Complete face-to-face visits to perform assessments and develop care plans, then follow patients through treatment while referring them to community-based organizations to address care gaps. Integrate prevention and screening recall systems into electronic medical records, assist with data collection and reporting, and compile information for program summaries. Work flexible hours to improve health outcomes and reduce disparities for low-income, uninsured adults. Bilingual English/Spanish with an associate's degree and three years of healthcare experience required.

Required Qualifications

  • Associate's degree in related field
  • Bilingual English/Spanish
  • Minimum three years' relevant experience within the health care system, clinic setting, or other community based organization
  • Equivalent related experience may be substituted for degree
