Job Summary
Conduct outreach and inreach activities to assist with intakes, provide education, and coordinate medical appointments for potential PEP/PrEP and HIV positive patients. Assess patient needs, troubleshoot insurance issues, and enter data into AIRS and the AIDS Institute Reporting System. Deliver retention and adherence counseling, facilitate services while maintaining confidentiality, and increase linkage to care for undiagnosed individuals. Support community education at health fairs and participate in interdisciplinary trainings. Report to the Program Manager of the H.I.P.P program within Community Healthcare Network.
Desired Qualifications
- High School diploma or G.E.D.
- two years' experience in community work
- patient navigation/linkage
