Job Summary
Conduct outreach and inreach activities to assess patient needs for PEP and PrEP services, assist with intakes, program enrollment, and service navigation. Provide education on Treatment as Prevention, coordinate medical appointments, troubleshoot insurance issues, and enter data into AIRS and the AIDS Institute Reporting System. Facilitate PEP/PrEP services while maintaining strict confidentiality and ensuring prompt access to medical care, partner services, and STD screening. Participate in health fairs, tabling, and joint activities with community agencies to increase awareness and linkage to care.
Desired Qualifications
- High School diploma or G.E.D.
- two years' experience in community work
- patient navigation/linkage
