Job Summary
Patient Navigator in the Emergency Department Screening Program (EDSP) facilitates routine opt-out HIV and syphilis screening, coordinates referrals and follow-up with community partners and clinics, educates patients on STI/HIV prevention and treatment, ensures linkage to care and adherence, maintains EHR documentation, and supports program monitoring and quality improvement, all while applying trauma-informed, harm-reduction–focused approaches and using AIDET best practices to improve health outcomes in the ED.
Required Qualifications
- Minimum of one (1) year experience working in a healthcare environment.
- Experience as Community Health Worker is a plus.
- Bilingual or multilingual abilities reflective of local community needs are preferred.
- Lived experience or strong connection to the communities served is highly valued.
