Patient Navigator - Belvidere and Stanhope, NJ

Greet patients, answer clinic phones, and assist them in understanding treatment options and resources. Serve as the essential link between patients and the care team by facilitating appointments for consults and support services. Coordinate multidisciplinary planning conferences, develop patient summaries, and document recommendations using standardized care protocols. Follow patients throughout the care continuum, including inpatient admissions, while collaborating with inpatient care management. Work with staff to collect data, track outcomes, and support strategic planning processes. Maintain positive working relationships with physicians, office staff, nurses, and social services. Assist with center stocking and Patient Ambassador team activities.
