Job Summary
Provide direct case management services to patients enrolled in the HIV care coordination program, acting as their main point of contact. Accompany patients to appointments, deliver health education encounters, and coordinate logistics for adherence reminders, transportation, and childcare. Observe and record patient self-administration of ARVs, assess for side effects, and deliver support in homes, community-based organizations, or other convenient locations. Maintain client records according to protocols and represent the agency at various venues to promote services. Work closely with doctors, nurses, and care team staff to ensure optimal health outcomes. Requires fluency in English, experience with diverse populations, and ability to work flexible hours including evenings and weekends.
Required Qualifications
- Associate's degree in human services or social work
- at least 12 months experience in patient navigation or similar field
- Strong socio-cultural identification with the target population
- experience in working with target population
- Fluency in one or more Asian language
- Excellent written and verbal communication skills in English
- Ability to communicate well with medical providers and support staff
- Ability to work well with diverse population
- Ability to handle multiple tasks and stressful environment
- Ability to work well within the organizational structure
- Ability to work flexible hours including some evenings and weekends
- Ability to use common office software (Word, Excel, Power Point)
Desired Qualifications
- certification as a peer worker by the NYS AIDS Institute Peer Certification
- case management certification by CUCS
- at least 90 hours of documented training equivalent to those required by the NYS Peer Worker
- Familiarity with eSHARE
