Patient Management Nurse

Manage patient inquiries, prioritize urgent situations, and route cases to providers while maintaining HIPAA-compliant documentation. Identify high-risk patients, establish educational touchpoints, and provide transitional care for recent admissions or ER visits. Assist with acute medical needs by triaging patients, obtaining orders, and following through on treatment plans. Optimize referrals, connect patients with health plans and community resources, and resolve healthcare barriers. Review provider scorecards to identify improvement opportunities and develop action plans. Schedule appointments and interact with patients both telephonically and in-person to foster rapport and encourage direct contact.
