Job Summary
Patient Liaison role coordinating patient care and communications in a home health setting. Responsibilities include answering patient-related calls, introducing new patients to the company, building relationships with referral sources, conducting patient satisfaction surveys, assisting with intake referrals and physician orders, participating in on-call rotations, and supporting office operations. Requires college degree and at least 2 years of customer service and healthcare administrative experience, with experience in insurance verification, referrals, and scheduling; strong communication, organizational and multitasking skills; ability to collaborate across departments.
Required Qualifications
