Patient Enrollment Specialist - Transitions of Care

Review and prioritize patient census; reach out to patients to explain services and enroll them into appropriate programs based on line of business; schedule initial and annual appointments; enter documentation into designated systems; multi-task across calls, texts, facsimiles, and electronic queues while taking notes and communicating with customers; demonstrate strong verbal and written communication, empathy, persistence, and attention to detail in a fast-paced, high-pressure environment; participate in hybrid/virtual work with in-person training sessions; ability to work from the office as needed; training and onboarding to develop essential skills in home and community-based care settings.
