Job Summary
Coordinate patient care through the clinic and manage service delivery efficiency: check-in/check-out, telehealth support, referrals and care coordination, and scheduling optimization. Facilitate collaboration between clinical and clerical staff to promote patient engagement and timely service delivery; monitor utilization metrics and adapt schedules as needed; support cross-training to cover for staff during absences; handle inquiries and assist with triage-driven scheduling. Proficient with EMR/EHR systems (e.g., OCHIN/EPIC) and standard office software; uphold professional, respectful communication and confidentiality; contribute to continuous process improvements and alignment with organizational mission. Expected to work across multiple Health Center locations as assigned.
Required Qualifications
- Must possess computer proficiency in Word, Excel, and the EMR (e.g. OCHIN/EPIC)
- Must understand and embrace a systems approach to service delivery, as well as agency and program mission
- Must have strong communication skills, be assertive, and able to engage patients and staff in building positive working alliances
- Office management experience strongly preferred
- Experience in monitoring outcomes data and performance indicators
