Patient Engagement Coordinator; Health Center Operations

Job Summary
Coordinate patient care through the clinic, including checking in, escorting to exam rooms, answering questions, and assisting with pre- and post-appointment services; support telehealth duties (virtual check-ins, scheduling, follow-ups); help providers and counselors manage client volume and access; monitor referrals via the EHR, manage wait times, and respond to inquiries; adjust schedules per clinical triage, analyze utilization metrics, and oversee scheduling of pharmacologic management and primary care providers; collaborate across clinical, supervisory, and clerical staff to ensure efficient, timely service delivery; cross-train to support Patient Support Staff during absences and potential location assignments.
Required Qualifications
- Must have high degree of initiative and orientation toward process improvement
- Strong organizational skills and critical thinking
- Proficiency in Word, Excel, and EMR (OCHIN/EPIC)
- Ability to engage patients and staff and build positive working relationships
- Team player with ability to work autonomously, creative, assertive, resourceful, and self-directed
- Experience in monitoring outcomes data and performance indicators
- Office management experience strongly preferred
