Job Summary
Conduct outbound phone outreach and home visits to engage patients and families with resources, care coordination, and appointment scheduling. Educate members on program services, health promotion activities, and available community supports while facilitating referrals and closing care gaps. Monitor patient progress through chart reviews, data administration, and screening tools to ensure timely access to primary care and social services. Collaborate with care teams and external partners to navigate complex situations, manage workloads, and maintain accurate documentation in electronic medical records.
Required Qualifications
- Associate's degree in relevant field
- At least two (2) years of relevant working experience
- Equivalent combination of education and experience
- Previous experience in a healthcare setting
- Working with vulnerable populations or community health
- Bilingual language proficiency
- Ability to read and interpret technical and other complex documents
- Ability to write routine correspondence such as letters and memos
- Ability to present information in one-on-one and small group situations to internal and/or external clients
- The ability to define problems, collect data, establish facts, and draw valid conclusions
- Ability to interpret an extensive variety of technical instruction
- Constructive thinking and alternative short-to-long term solutions
- Action plans must be developed and implemented
- Possesses basic operating knowledge of computers
- Intermediate level of Word, Excel and Outlook
- Willingness to learn any other electronic telehealth platform
- Previous electronic medical record experience
- Valid Washington State Driver's License
- Acceptable driving record determined by HealthPoint's insurance carrier
- MA-R credential
- Demonstration of ability and commitment to obtain the credential within six (6) months to one (1) year of hire
- Proof of vaccination for COVID-19
- Proof of immunizations and/or immunity to MMR (measles, mumps, rubella)
- Proof of immunizations and/or immunity to Varicella
- Annual Influenza immunization
- TB QuantiFERON Gold Titer
- Tdap (within last 10 years)
- Hepatitis B
- Proof of their immunizations and/or immune titer results prior to starting or no later than their fifth (5) business day of employment
Desired Qualifications
- Previous experience in a healthcare setting, working with vulnerable populations or community health preferred
- Bilingual language proficiency preferred
- Willingness to learn any other electronic telehealth platform
- Previous electronic medical record experience preferred
