Job Summary
Fully-remote Patient Coordinator coordinating between contracts, patients, HHAs, and care team; ensures effective communication, identifies issues during attendance verifications and introductions, and supports delivery of high-quality care. Responsibilities include scheduling based on Home Care Worker training, minimizing open cases, documenting communications in HHA Exchange, coordinating with patients, families, contractors, clinicians, aides, and other departments; informing on discharges and updates; completing daily/weekly/monthly reports; managing overtime/travel time; locating aides with Smart Map; completing incident reports; managing mutual/linked cases; handling emails, voicemails, and faxes promptly; and adapting to agency/industry changes. Candidates should demonstrate excellent customer service, multitasking, written and verbal communication, time management, teamwork, resilience under pressure, and flexibility.
