Job Summary
The Patient Continuity and Needs Advocate (PCNA) supports ChenMed’s mission by evaluating patient needs and connecting eligible individuals with health plans and community resources, driving retention and re-engagement, and collaborating with center leadership, brokers, and clinicians to ensure seamless care transitions. Requires strong customer service, knowledge of healthcare benefit services, proficiency with Salesforce and EMR systems, HIPAA compliance, and excellent written and verbal communication. Minimum 3 years’ experience, high school diploma or GED required; BA/BS preferred. Pay range is $17.00-$24.26 per hour; benefits, growth opportunities, and bonuses may apply. This on-site role is based in Temple Terrace, FL.
Required Qualifications
- High School diploma or GED equivalent required
- Minimum of three (3) years’ work experience; working with seniors a plus
- BA/BS degree preferred or additional experience may be considered in lieu of the required education on a year-for-year basis
