Patient Case Manager

Job Summary
Oversee and coordinate workflow processes for new and existing referrals, reviewing status within 24-48 hours of receipt. Identify and resolve care coordination complaints from Department Managers, Sales/Marketing Staff, and Referral Sources. Monitor daily workflow queues to assess stop gaps and collaborate with Intake staff to streamline processes. Research causes of slow referral movement and assist in developing new tools to improve the referral workflow. Requires a High School Diploma or GED, with a current Licensed Practical Nurse or Certified Pharmacy Technician credential preferred. Experience managing high-volume complex patient cases in an Intake/admissions referral process is essential.
Required Qualifications
- High School Diploma or GED
- A current, unrestrictive Licensed Practical Nurse or Certified Pharmacy Technician for the State of Practice
- 3 or more years of experience in a pharmacy, healthcare or clinical setting managing a high volume of complex patient cases within an Intake/admissions referral process
- Proficiency in computer skills, including MS Office, Excel, and Word
- Knowledge and understanding of all aspects of the referral workflow process
