Patient Care Technician - PCT CCHT - Dialysis

Job Summary
Welcome assigned patients, perform vascular access cannulation, and initiate dialysis treatment according to prescribed orders. Monitor patients' response to therapy, obtain pre- and post-treatment vital signs, and evaluate vascular access for patency. Calculate and enter individual dialysis machine programming for ultrafiltration goals and document all treatment parameters on the Hemodialysis Treatment Sheet. Assist in patient education regarding adequacy measures, disease processes, and home dialysis options, while ensuring strict adherence to infection control policies and equipment safety checks. Follow newly admitted patients through their first four weeks in the Transitional Care Unit, coordinating their transition to chosen dialysis modalities.
Required Qualifications
- High School diploma or G.E.D.
- Must meet Center for Medicaid/Medicare Services (CMS)-approved state and/or national certification requirements within the required state or CMS timeline
- All appropriate state licensure, education, and training (if any) required
- Demonstrated commitment to organization culture, values, and customer service standards
- Previous patient care experience in a hospital setting or a related facility preferred
- Successful completion of the organization dialysis training program
- CPR certification
