Job Summary
Admit, discharge, and transfer patients while completing appropriate forms. Provide patient hygiene, food and fluid needs, and mobility assistance. Obtain and record vital signs, perform finger stick blood glucose monitoring under RN direction, and maintain a safe environment including pressure ulcer prevention. Complete documentation accurately according to hospital policies. Support patient rights regarding cultural, ethical, and religious diversity while adhering to standard precautions and infection control standards. Requires current Nurse Assistant certification, BLS certification, and high school diploma. Preferred experience includes long term care, rehabilitation, and medical surgical settings.
Required Qualifications
- High school diploma or equivalent
- Current Nurse Assistant certification by the State of Illinois
- documentation of previous Nurse Assistant certification within the past 5 years
- documentation of on the job training in a patient care setting equivalent to nurse assistant training
- BLS certification
