Patient Care Navigator

Care Navigator responsibilities include managing administrative functions for a team of 200 homebound patients and their providers (Nurse Practitioners or PAs), ensuring care needs of a complex homebound population are met through exceptional customer service, coordinating timely visits, and communicating effectively across care teams. Build trusted relationships with providers, home health and hospice agencies, patients and families, and Bloom leadership. Own team performance measures related to provider productivity, patient satisfaction, and timely call responses. Balance referrals, visits, records, and supply requests with strong time management. Ensure timely patient visits as requested by providers, and optimize provider schedules to maximize effective driving routes. Additional duties include creating provider schedules, answering calls, confirming appointments, scheduling urgent appointments, processing incoming faxes, requesting patient records, ordering DME and oxygen, and sending referrals to specialists, home health or hospice as requested. Key skills include current healthcare experience, leadership, multi-tasking, problem-solving, excellent communication, and familiarity with EMR (eCW preferred).
