Job Summary
Home Health Patient Care Manager responsible for the overall supervision and coordination of clinical services. Coordinates and supervises an interdisciplinary team to ensure continuous high-quality care to home health patients in accordance with physician orders and the plan of care, and all applicable state and federal laws. Responsibilities include handling referrals, clinician assignments, start-of-care plotting, determining patient home health benefits and medical necessity, ongoing insurance approvals, ongoing patient assessments, and ensuring care is tailored to patient needs and compliant with eligibility criteria. Lead daily reviews of assessments and plans of care, consult with clinicians as appropriate, and navigate expectations across the care team while maintaining accessibility and equity in care. RN licensure and ability to work within diverse patient populations are essential.
Required Qualifications
- Current unrestricted RN licensure in state of practice
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation
