Patient Care Manager - RN

Direct and coordinate clinical departments and sites, assuming responsibility for continuity, quality, and safety of services delivered in compliance with State and federal regulations. Supervise nursing staff to ensure quality, Plan of Care adherence, and appropriate assessment of patient needs. Support employee and patient education through continuous training, coaching, development, and annual evaluations. Ensure timely documentation submission and overall compliance with local, state, and federal laws, including Medicare regulations and established personnel policies. Perform other duties as required while maintaining continuity of care for patients with life-limiting illnesses.
