Job Summary
Conduct home assessments to determine consumer eligibility for Home Care services and supervise a team of Care Team Associates. Effectively communicate with Direct Care Workers and patients regarding service updates, while leading and observing contact visits to ensure quality and compliance. Advise household members on practical care tasks, complete routine well-being updates, and assist with onboarding new patients and caregivers. Meet operational growth goals and foster an engaged workforce while ensuring all services meet Company standards and legal requirements.
Required Qualifications
- Previous experience in home care, nursing or other medical fields
- Associate's degree or Bachelor's degree required
- Ability to build rapport with patients
- Strong work ethic
- Strong communication skills
- Ability to work flexible hours to meet patient and employee needs
Desired Qualifications
- Two or more years of healthcare leadership preferred
- Bilingual preferred
