Patient Care Manager

Lead and supervise a team of Care Team Associates to ensure patients receive personal care services based on their needs. Meet operational and growth goals, including key performance metrics, while ensuring quality of services delivered meets Company standards. Foster an engaged workforce and ensure compliance with the law and Company policies. Assist Care Team Associates with day-to-day duties, including screening new patients and caregivers, on-boarding, staffing patient cases, communicating with patients and families to ensure satisfaction, and conducting on-site visits as needed. Completeive pay is paid weekly with medical/dental/vision/life insurance, paid holidays, PTO, and a 401(k) match. An Associate's or Bachelor's degree is required, with two or more years of healthcare leadership preferred. Bilingual candidates are preferred.
