Job Summary
Lead and supervise a team of Care Team Associates to ensure patients receive personal care services based on their individual needs. Meet operational and growth goals while maintaining quality standards and fostering an engaged workforce. Assist Care Team Associates with day-to-day duties, including screening and onboarding new patients and caregivers, staffing patient cases, and conducting on-site visits. Communicate regularly with patients, caregivers, and families to ensure satisfaction and service delivery. Ensure compliance with laws and company policies. Requires an Associate's or Bachelor's degree with two years of healthcare leadership experience; bilingual skills preferred.
