Job Summary
Specialist, Patient Care Delivery provides in-home clinical care to health plan members, traveling to multiple member homes (up to 120 miles daily) to conduct visits, collect vitals, support in-home assessments, administer injections and blood draws, manage mobile supplies, and document care in EHRs. Responsibilities include educating members on care plans, maintaining HIPAA-compliant privacy, coordinating scheduling and referrals, serving as a field liaison to the care team, and supporting billing workflows. Required licenses/certifications (MA/CNA/LVN/LPN/Phlebotomist) and CPR, with travel-ready transportation; preferred experience with Medicaid/Medicare populations and mobile EHR systems; bilingual abilities encouraged.
Required Qualifications
- Active license or certification as one of the following: Medical Assistant (MA), Certified Nursing Assistant (CNA), Licensed Vocational/Practical Nurse (LVN/LPN), or Phlebotomist
- Experience-based clinical proficiency aligned with credentialed role
- Completion of training program applicable to credential
- Current CPR certification
- Valid and unrestricted driver's license, reliable transportation, and adequate auto insurance for job related travel requirements
- If not currently certified for phlebotomy, must obtain certification within 6 months of hire
- Outstanding communication and interpersonal skills
- Ability to work independently and adapt to varying home conditions
- Knowledge of clinical safety, infection control, and emergency response
- Comfort using technology for documentation, scheduling, and coordination
- Commitment to continuous professional development and member-first care
- Bilingual (English/Spanish or other language relevant to member population)
- Experience with Medicaid/Medicare populations
- Familiarity with mobile EHR systems (e.g., Epic, QNXT)
- Prior experience in home health or mobile care delivery
