Patient Care Coordinator (WIVA)

Patient Care Coordinator will provide lead-level coordination and communications to support interventional and vascular office patients and referring physicians. Responsibilities include answering multiple phone lines, navigating multiple electronic systems, delivering top-notch customer service to patients and referring physicians, securing demographics and documentation, validating information for completeness and accuracy, maintaining PHI/HIPAA confidentiality, evaluating information for standards compliance, documenting information in computer systems, coordinating patient through the continuum of care, supporting departmental and corporate plans, meeting strict deadlines, managing time, and handling conflict with patients and staff. The role requires teamwork with physicians and staff, flexible responsiveness to changing demands, and clear, concise communication.
